Pre-Employment Forms
IMPORTANT: Employees MAY NOT begin working until the criminal background check and sex offender registry check have been completed, and the supervisor has been notified by HR.
New & Returning Employees
For new employees and returning/retiree employees if separated from DMACC for at least one year. The following pre-employment forms must be completed and approved by Human Resources:
- Request for Adjunct, Temporary or Student Employee (P-61A)
- Authorization/Consent & Release for Background Check (P-61B)
- Data Record for Regular Employees (P-13) or Data Record for Credit Adjunct Instructors, Temporary & Student Employees (P-14) (A Data Record is not required for Non-Credit Adjunct Instructors.)
- Course Approval for Instructors (P-24) (adjunct instructors only)
- Employee application and transcripts (full time employees and adjunct instructors only)
